Ankle Sprain: More Than Just a "Twist"

Ankle sprains, commonly known as a "twisted ankle," are a very common everyday injury. They can happen from stepping awkwardly while playing sports or from an uneven surface while walking. Many people think a twisted ankle is a minor issue that just needs some ice. However, without proper treatment, it can lead to chronic pain and affect your life in the long run.

What is an Ankle Sprain?

Ankle sprains are generally classified into three types:

第一級扭傷(輕度)

Grade 1 (Mild):

The ligament is slightly stretched but not torn. There is usually only minor pain and swelling, and walking is not significantly affected.

第二級扭傷(中度)

Grade 2 (Moderate):

The ligament is partially torn. This causes moderate pain, swelling, and bruising, with difficulty or instability when walking.

第三級扭傷(嚴重)

Grade 3 (Severe):

The ligament is completely torn. There is severe pain, significant swelling, and bruising, and the ankle joint is completely unstable and unable to bear weight.

Causes and Symptoms

Causes:

The most common cause of an ankle sprain is an inversion sprain, where the ankle rolls inward, and the sole of the foot turns inward. This often occurs in the following situations:

  • Walking or running on uneven surfaces
  • Landing awkwardly after a jump
  • Making quick changes in direction during sports

Common symptoms of an ankle sprain include:

  • Pain: A sharp, immediate pain at the time of injury, which then continues.
  • Swelling: The injured area swells up quickly.
  • Bruising: Blood vessels rupture inside, causing bruising on the skin's surface.
  • Limited Mobility: Pain and swelling reduce the ankle's range of motion, making it difficult to bend or straighten.
  • Instability: The ankle feels loose or unable to support your body weight.

Treatment and Rehabilitation

  • Rest: Stop all activity and avoid putting weight on the injured ankle.
  • Ice: Apply an ice pack to the injured area for about 15-20 minutes at a time, several times a day, to reduce swelling and pain.
  • Compression: Gently wrap the ankle with an elastic bandage to provide support and help reduce swelling.
  • Elevation: Elevate the foot above heart level to help with blood flow and decrease swelling.

However, rest alone is not enough for a full recovery. Professional physical therapy and rehabilitation are crucial for treating an ankle sprain. They not only speed up recovery but also help prevent future injuries. A physical therapist will use manual therapy and electrical modalities to help with pain and inflammation and will create a personalized rehabilitation plan to gradually restore the ankle's range of motion, strength, and stability.

1. Stretching Exercises

Towel Stretch

Sit with your legs straight out in front of you. Place a towel or resistance band around the sole of your foot. Gently pull the towel toward your body until you feel a stretch in your calf and the back of your ankle. Hold for 15-30 seconds and repeat 3 times.

Ankle Circles

Sit or lie down and slowly rotate your ankle inward, outward, upward, and downward. Complete 10 circles clockwise and 10 circles counter-clockwise.

2. Strengthening Exercises

Resistance Band Ankle Training

Secure a resistance band around a table leg or door handle, and loop the other end around your foot. Perform inversion, eversion, dorsiflexion, and plantarflexion (upward and downward) movements. Complete 10-15 repetitions for each movement, repeating for 3 sets.

Single-Leg Stand

Stand on a flat surface on one foot, maintaining your balance for 30 seconds. If this is too easy, try closing your eyes or standing on an unstable surface (like a pillow or balance pad).
